Health Insurance: (UMR- Subsidiary of United Healthcare)

All full-time employees are eligible. The effective date of coverage is the first day of the month following completion of 90 days of service. Officers at the AVP level or higher have an effective date as of the first day of employment.

Health Insurance is in the form of a Preferred Provider Organization (PPO). Employees may elect to enroll their dependents. Whether employees select to cover themselves or their family, Community Bank of FL contributes substantially to the cost of coverage.

Dental Insurance: (UMR- Subsidiary of United Healthcare)

All full time employees are eligible. The effective date of coverage is the first day of the month following the completion of 90 days of service. Officers at the AVP level or higher have an effective date as of the first day of employment.

Community Bank of FL offers a preferred provider plan. The insured employee may receive treatment from any dentist. There is a $100.00 deductible for both individual and family coverage, and basic services are covered at 80%.

Life Insurance Plan

All full time employees are eligible. The effective date of coverage is the first day of the month following completion of 90 days of service. The life insurance program provides term life and accidental death & dismemberment insurance for each employee at 2 times base annual salary; 100% paid by Community Bank of FL. Employees can choose additional group life for themselves, their spouse and/or children at 100% cost to the employee.

Long-Term Disability Plan

All full-time employees are eligible. The effective date of coverage is the first day of the month following completion of 90 days of service. Employees are insured against loss of wages as a result of a catastrophic illness or injury. After 3 months of disability, the plan provides a benefit equal to 60% of the disabled employee’s salary up to $10,000 a month maximum. Community Bank of FL pays the full cost of this benefit.

Flexible Spending Plan:

Once an employee has met the eligibility requirements to enroll in the Community Bank of FL health insurance program, they may also choose to participate in the Flexible Spending Plan.

The HealthCare Flexible Spending plan enables an employee to designate a deduction amount from each paycheck to place into an account to cover out- of-pocket medical costs incurred for the plan year.

The Dependent Flexible Spending Plan enables an employee to designate a deduction amount from each paycheck tax free to place into an account to cover out-of-pocket dependent day care costs for children under the age of 13 or elderly care.
